Insurances Accepted

We currently accept Medicare clients, most Preferred Provider Organization's (PPOs) and private pay clients. We can do a free Medicare eligibility over the phone within minutes. We can also guide you in your rights and what are some possible solutions for your specific and very unique circumstances. 

When calling us for a free Medicare eligibility request, please have the following ready:

  • Patient's full name (as written on the Medicare Card)*
  • Patient's full Medicare Number (including the letter(s) following the Medicare Number)*
  • Patient's Date of Birth
  • Patient's Gender

*  Please note that in some circumstances, a client's Medicare benefits may be under a deceast spouses Medicare information.

Insofar as private pay clients, please call us and we can discuss specific rates which may vary depending on the type(s) of services requested as well as the location of the client's physical location. Our rates are based on Medicare rates and when you pay a retainer, we will significantly reduce our rates to facilitate the care process.
